Welcome to Summerfield Health and Rehabilitation Center, where exceptional health care and the experience of our staff combine to create one of the most trusted health care facilities in the region. Serving Louisville’s south end for more than 30 years, we offer services such as Short-Term Rehabilitation Services to include Physical, Occupational, and Speech Therapy, 24-hour Skilled Nursing Care and Long-Term Care. Our residents originate from neighboring towns such as Pleasure Park Ridge, Okolona, Shively and Valley Station. Some residents require a short stay following an illness, injury or acute hospital stay. Others require ongoing quality long-term care and support.

You'll find the atmosphere is nurturing, warm and inviting. The vaulted ceiling and skylights of our one-story building creates a bright, open feeling. Three enclosed courtyards welcome you with flower gardens, patio furniture and park benches. There are four tastefully furnished lounge areas ideal for relaxing or socializing with friends and family. A variety of tempting menu selections are available for each meal, served in one of our cozy dining rooms.

Expert health care is personalized to meet your individual needs and provided by experienced medical professionals. Should more comprehensive care become necessary, it is available right here. Our own staff therapists provide rehabilitation services with patience and encouragement.
